Takeda stapt (in navolging van Nkarta) met haar CAR-NK (CD19) programma over van Non-Hodgkin naar auto-immuunziekten:twitter.com/PearlF/status/17886291705... Ook schrapt Takeda een aantal CAR-t programma's. De CAR-t markt lijkt me inderdaad totaal overbevolkt. Van haar CAR-NK programma (in licentie van Anderson) wisten we toch al dat Anderson naderhand heeft aangegeven waarom dit niet aan de verwachtingen voldeed waarna ze zelf met een verbeterde versie is gekomen. Op zijn "best" krijgen zo de NK engagers van Affimed alle ruimte van de markt.www.europeanpharmaceuticalreview.com/...

Lijkt me nogal frustrerend gezien ook alle bijwerkingen. Is CAR-NK dan uiteindelijk beter of blijft Anderson daar nu de enige die doorgaat?

Volgens bovenstaand artikel is dat een optie omdat bewezen is dat pd1 blokkers bij Hodgkin erg werkzaam blijkt te zijn. Gelukkig is er geen commerciële cd30-CAR-t product op de markt of in ontwikkeling. Resultaten dáárvan in de VS en China in studies lijken tegen te vallen. AFM13 wordt wel genoemd in bijgaand artikel. Zo te zien is AFM13 wel degelijk interessant voor een Roche of Merck.

Anderson herbergt meer brains dan de rest bij elkaar. Op zich is dit goed nieuws voor Affimed omdat Anderson zich concentreert op non-hodgkin (CD19) en niet op hodgkin (CD30) zoals Affimed doet. Mbt de duurzaamheid wordt het volgende opgemerkt: "A follow-up published in Nature Medicine in January 2024 showed that some patients maintained their remission for at least a year after treatment. This suggests that CAR NK cells are continuing to look for cancer cells.? “Our hope is to provide treatment that results in long-term remission,” notes Rezvani. “But we only have data for one year after the clinical trial. Ik heb inderdaad veel geduld met deze toko, voornamelijk omdat ze in mijn visie een baanbrekend medicijn tegen hodgkin en acute myeloide leukemie lijkt te hebben. De rest is alleen maar afleiding. De data zijn toch om 11 uur Nederlandse tijd vrij gegeven? Het is geen uitslaande brand in de nabeurs handel maar een gemiddelde PFS van 5,9 maand tot heden lijkt me niet slecht als je je bedenkt dat al deze patiënten in feite uitbehandeld waren. Jammer dat er vergeleken bij de januaridata geen partial of complete response bijgekomen is. Per 1 juni worden ook data gekend gemaakt mbt het Mutant cohort. Die data zouden nog beter kunnen zijn.

One patient showed a confirmed complete response, and three patients showed confirmed partial responses (PR). In addition, seven patients achieved stable disease, resulting in a disease control rate of 73.3% (11/15 patients). Median progression-free survival was 5.9 months. All responders were resistant to checkpoint inhibitor treatment prior to the study, which supports the hypothesis that combining AFM24 with atezolizumab may enhance the cancer-immunity cycle and provide an alternative strategy to overcome resistance to existing therapies for EGFR-expressing tumors. AFM24 and atezolizumab were given at their respective single agent doses. Treatment in these heavily pretreated patients was well tolerated. Side effects were consistent with the known safety profiles of these agents. The most frequent side effects observed were mild to moderate infusion related reactions and transient mild to moderate increase in liver enzymes. “We are very encouraged to see objective and lasting responses in patients who have failed multiple lines of therapy including platinum doublets and checkpoint inhibitors,” said Dr. Andreas Harstrick, Chief Medical and acting Chief Executive Officer of Affimed. “These data support our hypothesis that the combination of AFM24 and PD-1 targeting may act synergistically on the immunity cycle. Wat biedt de concurrentie dan? Stable disease telt in deze rekenwijze helaas niet als response maar wat voor opties hebben deze patiënten ? Bij PD1 blokkers en conjugaties gaat het het in 100% van de gevallen ook om het rekken van het lijden met enkele maanden en die hebben meer bijwerkingen dan AFM24 lijkt me.
